Restaurant inspections: critical violations April 28

The following restaurant inspections with critical violations were conducted by Richland Public Health between April 28 and May 4:

Temple Christian School, 752 N. Stewart Road, Mansfield, April 28. Unnecessary persons in food prep or storage areas (critical, corrected during inspection). Two persons not on food staff were observed walking through kitchen during lunch period.

Outback Steakhouse, 820 Lexington Springmill Road, Mansfield, April 28. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed container of butter in Arctic Air counter cooler at prep table to have internal temperature of 46 degrees. PIC placed butter in larger reach-in cooler to cool to 41 degrees.

Wilging's Fine Meats, 1391 Lexington Ave., Mansfield, April 28. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed prepared ham salad to be date marked April 17, four days past use. PIC voluntarily discarded ham salad. Presence of insects (critical, corrected during inspection). Observed live ants along the caulking of the warewashing unit. Continue to monitor the area for continued activity and, if noted, contact licensed pest control company. PIC removed all evidence and cleaned and disinfected the area.

Taco Bell #30674, 1194 Park Avenue West, Mansfield, May 2. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed sanitizer bucket by the middle hand sink to have a concentration below 150 ppm. Ensure sanitizing solution reach a concentration between 200-400 ppm.

Mickey Mart #21, 750 Ohio 97 W., Bellville, May 2. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). PIC unable to locate signed employee illness reporting agreements. Provided copies of Richland Public Health's employee illness reporting agreement during inspection and PIC signed copy.

The Golden Burrito #2, 1740 Park Avenue West., Ontario, May 3. TCS foods not being cold held at the proper temperature (critical repeat). Observed cooked pork in reach-in prep cooler with internal temperature of 46 degrees and date marked for prep on April 29. Observed cooked chicken in reach-in cooler made May 2 with internal temperature of 43-44 degrees. PIC voluntarily discarded 2.5 pounds of pork and 2.5 pounds of cooked chicken during inspection.

Crestview Middle School, 1575 Ohio 96 E., Ashland, May 3. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Probe thermometer was not being sanitized before and after measuring temperature.

Doc's Deli, 424 Glessner Ave., Mansfield, May 3. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed uncovered ice cube trays used for drinks in the freezer. Missing and/or incomplete allergen declaration(s) (critical repeat). Containers of lemon puffs and old fashioned cake in the retail grab-n-go cooler did not have list of ingredients or allergen statements posted. Correct by May 10.

Joe & Mary's Market, Adm. Sturges Inc., 162 Sturges Ave., Mansfield, May 3. Presence of rodents (critical). Observed rodent droppings behind the mop sink in the storage area and behind shelving and ice cream freezer near the walk-in dairy cooler door. Remove all evidence of rodents and disinfect areas; continue to monitor for activity. Correct by reinspection May 10.

Keat's Coffee - Richland Mall, 615 Richland Mall Drive, Mansfield, May 4. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Provided Richland Public Health's (RPH) employee illness reporting agreement; both PIC and food employee signed during inspection. Ensure illness reporting agreements are maintained in facility. Handwashing sink not accessible (critical repeat, corrected during inspection). Observed utensils sitting inside hand washing sink. Presence of live insects (critical). Observed small flying insects in restroom. Clean restroom and rid of pests by May 11.

Shell #1357, 1357 Lexington Ave., Mansfield, May 4. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC stated facility uses a bucket with detergent, chlorine and water to wash items, rinse the items, and then towel dry in only one compartment of the three-compartment. Ensure the washing process occurs in separate compartments of the three-compartment sink. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Observed mop handle on handwashing sink. Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired (critical, corrected during inspection). Observed two, two-pack of baby food puree to be expired. PIC voluntarily disposed of the baby food.

Smokin' Bros BBQ, 1041 Park Avenue West, Mansfield, May 4. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed sanitizer bottle at a concentration of over 500 ppm. Ensure sanitizer has a concentration of 200-400 ppm. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed potato salad and cole slaw not date marked. PIC verified that product as made/opened on April 29. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed no common name on the dish detergent spray bottle. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed scissors that were not easily separable for easy cleaning being used for food packages. PIC stated plans to get new scissors; discontinued use of scissors for food packages.

Pretzelmaker (Richland Mall), 2208 Richland Mall, Space #c-8, Mansfield, May 4. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed bucket of sanitizer solution in front area to be 500 ppm.
